Form Consent & Electronic Submission Agreement

1. Legal Recognition of Electronic Signatures (R.A. 8792, Sec. 8)

Under Section 8 of the Philippine Electronic Commerce Act of 2000, electronic signatures carry identical legal weight, enforceability, and validity as handwritten ink signatures. By:

• Clicking "Create Account" on our registration forms;
• Clicking "Confirm Reservation" or "Show Available Times"; or
• Selecting electronic acknowledgment checkboxes,

you provide your valid electronic signature affirming your agreement to abide by platform rules, venue safety guidelines, and payment commitments.

2. Voluntary & Informed Consent for Data Processing (R.A. 10173)

Pursuant to Section 12(a) of the Data Privacy Act of 2012, you grant freely given, specific, and informed consent for JazzuCourt to process your personal data (full name, phone number, email address, reservation logs) exclusively for:

• Authenticating your membership credentials and securing your account.
• Synchronizing court slot availability and generating unique digital check-in receipts.
• Transmitting necessary player check-in rosters to participating sports venue managers in Balangkayan.
• Transmitting critical transactional SMS or email alerts regarding booking schedules and weather alerts.

3. Sports Gameplay & Physical Activity Consent

When reserving a court for pickleball, badminton, basketball, volleyball, or other sports through JazzuCourt:

• Physical Fitness: You affirm that you and participating players are in adequate physical health to engage in athletic activity.
• Voluntary Assumption of Risk: You acknowledge that sports activities carry ordinary risks of physical strain or injury. JazzuCourt provides reservation software and is not liable for personal accidents occurring on court premises.
• Venue Compliance: You agree to follow all safety, footwear, and equipment guidelines established by individual facility owners.

4. Consent on Behalf of Minors

If reserving court time or creating an account on behalf of minors under 18 years of age, the form must be completed by a parent, legal guardian, or authorized team coach who gives explicit consent under Philippine law for roster recording and assumes supervisory responsibility.

5. Revocation of Consent
